Welcome to American Fork City’s online resource for submitting non-emergency service requests to the City.  Requests for action by Public Safety should be directed to 801-763-3020 for City Police or 801-763-3045 for the Fire/Rescue Department.  For all emergencies call 911. 

If you live in, work in or are visiting or do business with American Fork City and you have a service request, you can use the Citizen Request Services System to notify us of a problem or service concern. The request automatically gets routed to the appropriate service area for action.  By providing your contact information within the Citizen Request System, you can visit the site 24/7 to check on the status of your request.  In addition, entering your contact information allows us to communicate with you.

This online module is generally to be used to request service with regard to public maintenance and to track the progress of the request or service.   Typically requests may include fixing sinkholes, potholes, repairing sidewalks, maintaining trees on City property, missing road signs general complaint, etc.
